Definition
The PKM (Personal Knowledge Management) failure pattern describes why systems for capturing and organizing personal knowledge consistently collapse: the bottleneck is not capture but retrieval, and retrieval requires active, ongoing maintenance that humans cannot sustain long-term. The user is positioned as the maintenance agent — the person responsible for tagging, linking, organizing, and surfacing — and humans are structurally bad at this role.
The gap between capture and retrieval is where every PKM system dies. Notes accumulate. Connections don’t. The system becomes archaeology.

- The failure pattern may be less about the architecture and more about mismatched expectations: PKM systems are sold as “second brains” but function as structured archives
